We require an evaluation of your dog prior to being admitted into our dog daycare program. This is for the safety and well-being of all the dogs in our care. Evaluations are done Monday through Thursday by appointment and are free of charge.
Please read, completely fill out, sign and save the following forms. Forms can be emailed to email@example.com, or they can be printed and faxed or dropped off at our facility. Our FAX number is 407-295-3889.

Because we only take 10 to 12 dogs per day, daycare is by reservation only. With our membership option, your dog is guaranteed a spot on the day(s) you select. With our package option, space is on a first come, first serve basis. You must call ahead to reserve a spot.
All dogs must have a current vaccination record on file with us. We require Rabies, Distemper/Parvo (DHPP or DAPP), and Bordatella. These vaccines must be current according your vet’s policy. We also require a Fecal exam that must be current within six months. Heartworm exams must be current within a year and dogs must be on a heartworm preventative. We currently don’t require Canine Influenza. We will accept titers for all vaccines except Bordatella. Dogs must also be on a flea / tick preventative. Please bring your dog’s vaccine record to your evaluation. You can either email your intake forms or bring them with you to your evaluation.
Daycare is currently available Monday through Friday only. Drop off is any time after 7:30am and pick-up is any time before 6:30pm. Late fees apply when dogs aren’t picked up before 6:30pm. Dogs that are not picked up by 7pm will be boarded for the night at Rocky’s Retreat. You agree to pay for all associated charges.
While we accept all major credit cards, cash and personal checks as forms of payment, we require a credit card to be on file.
We ask for 24 hours notice for daycare cancellations, but understand that sometimes emergencies happen and you must cancel with less notice. If this happens, please give us as much notice as possible. Failure to call us ahead of time will result in you forfeiting the day.
Please ensure that your dog is wearing a quick release collar. If you like, you may also bring lunch or a blanket for your dog.
We are not responsible for lost or damaged items.
Our lobby and daycare will be closed in observation of the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. We reserve the right to also close the lobby and daycare the day after Thanksgiving, Christmas Eve and New Year’s Eve. On days when the lobby is closed, we are still staffed for boarding dogs.
